Yeah when it comes to "donks" notes are essential

I like to use hard boiled egg classifications, and if I come across a really really good player, uhhh ummm one thats better then me, which is rare, but if I do, I write deviled egg. A kind poker player , one that allows me to take his blinds and never puts up a fight. I call them good eggs.

How many hands do you guys think is appropriate before you can make accurate notes on someone?

I try to never limit myself to no less than 5 sessions which include nothing less than 100 hands per session with each individual player before exacting notes on a player.

I always include the type of tourney when I jot down a note - I've been damaged by using notes from freerolls when I meet a player in a real-money game.
