There many variables that go into folding or betting, you cant really sum up a rule for when to fold, bluff or bet. You just have to keep in mind whats on the board and how strong your hand is. If you have pocket 3's and the flop is 3,2,4 then you have to think to yourself weather you think you have a strong hand against whoever is in the pot, while also taking pot odds into consideration( Pot odds are say your last to act and three people before you call the bet then your getting 4 to 1 on your call). Generally if I had a weak pocket pair Ill tend to play passive with it and if you hit trips with your weak pair Id say just through out a tiny bet and see how your oppents react to it then act from there.

I sometimes make the mistake of calling just to see what the other person has.. So I can use that later on (Especially in a tourney)

you will usally get called 75% of the time post-flop mainly because more people in the pot... the idea is not to throw much at it when your bluffing to keep you afloat if you get caught.... a call either signifies a chasing player (so keep you eyes out for flushes or straights) or someone with a already made hand and are relying on you to hang your self with commiting more and more chips in the pot.

after the turn when if still looks tastey to try to take the pot down... well do just that do another raise (the size of the raise depends on their chips stack, your chip stack, blinds and pot size) you will get called 40% of the time about here... so keep your eyes open to what hands he or she is trying or has made... i find trips the hardest to notice)

you called your down on the river card, either a simple check here will do if you know your beat or if you feel there is a busted flush or straight draw on the table assert pressure on them fire a 3rd bullit on them either from 1/2 pot to all-in (all-in is a last resort if your oppenent has a bigger stack than you and if there short stacked as they may just call you anyway) all-in should be used on a smaller stack than yours so it would put them out but leave you in shape to re-build your stack.

if caught bluffing stay low and play tight for a while to build your rep up again

I know when to fold when the odds are REALLY against me...I don't keep track of my opponents even though I know I should, this can't help a lot in an uncertain situation

I think itīs depends...If itīs a live game is easier to know a bluff, but online itīs very difficult...Zab, what software do you recommend to "know your oponent"?

I just hate low PPs, and if I hit a set on the flop, I feel like I am going to be all in no matter what.
